ENROLLMENT 2011-2012 To enroll, the parent/guardian must: 1. Schedule a testing and interview session. A non-refundable testing fee of $50 is due at this time. Returning students need not be retested each year. 2. Fully complete the application and return it to the school. Current copies of the child’s report cards, achievement tests, medical/immunization records on school forms, three point screening, a copy of the child’s birth certificate, and his/her social security card are also required. 3. Complete and sign the Tuition Form and Policy and Procedure Form which explains tuition payments and school policies. Signing these forms constitutes a contract between Academics Plus and the parent/guardian for the payment of tuition for the full academic year. 4. Pay the Registration Fee of $250 for pre-k and kindergarten, or $300 for first through fifth grade. This must be paid at the time of enrollment to reserve your child a place and to order curriculum textbooks. The registration fee is non-refundable.
